AAKP believes fellow patients move back and forth along the continuum of phases and within each phase there are health and life issues that need to be addressed by the patient, family members and healthcare professionals.
The AAKP Patient Plan© is designed to address questions and concerns at the various phases of the disease process. We have educational materials that address:

2 ACCESS AND INITIATION
3 STABILIZATION
4 ONGOING TREATMENT
The AAKP Patient Plan© has three major components:
1. Patient/Caregiver Check Lists
2. Phase Books
3. Phase Newsletters
Each phase includes a book of educational materials and checklists as well as a phase newsletter containing information related to the specific stage. Each phase also includes a postage-paid return card to request the next phase when the patient or family member feels they are ready and want to gather information along the continuum of care.
